L ABNORMAL OCCURRENCE REPORT Big Rock Plant 1.

3 Facility: Big Rock Point Plant.

Identification of Occurrence:

Energency diesel generator test start failure.

5 Conditions Prior to Occurr'nce: Plant in cold shutdown for maintenance e

and modification activities.

Description of Occurrence: The diesel failed to start properly in the test mode during the weekly test. Two complete cranking cycles did not start the unit. Following hand pump priming of the fuel lines, the unit started satisfactorily. Subsequent test starts on April 10, April 11 and April 17 vere normal.

7 Designation of Apparent Cause of Occurrence: Exact cause of failure is still unknown. However, all evidence supports a fuel supply deficiency and investigation of components is still in progress.

Analysis of Occurrence The unit was unserviceable for less than 30 minutes. The plant was in cold shutdown condition and both the 138 kV and the 46 kV transmission lines in service, thus no undue hazard existed during this period.

9 Ccrrective Action: The unit was immediately startel and manual priming characteristics were verified to be normal. Special tests were satis-factory as shown in Item 6.

Further corrective act:4ons will depend on the results of an investigation currently under way.

This investiga-tion includes the fuel supply system components that could contribute to introduction of air or loss of adequate fuel flow and consideration of additional monitoring facilities on the pump head tank.

Failure Data: Fuel supply and associated governor lubrication problems were previously reported in our April 21, 1971 letter to P. A. Morris and a fuel pump supply problem was reported in our letter of June 10, 1974.
