Q ABNORMAL OCCURRENCE REPORT A0-16-75 1.

Report No: A0-16-75, Docket 50-155 2A.

Report Date: July 17, 1975 May 30,1975 (Classified As A0 on July 8,1975) 2B.

. Facility: Big Rock Point Plant, Charlevoix, Michigan h.

Identification of Occurrence: Procedural deficiencies associated with reactor and fuel pit valve (CV h027) inspection and repair.

5 Conditions Prior to Occurrence: Plant shut down for modifications associated with A0-1-75 6.

Description of Occurrence:

Quality Assurance Department audit of safety-related maintenance activities on June 13, 1975 identified deficiencies in sign-off of portions of the procedure and these deficiencies were classified as nonreportable at that time. Subsequent investigation and review by the Plant Review Committee on July 8,1975 identified that the procedure used (MGP-h, Rev 0) was generic in nature and does not provide 4

adequate detail to cover the adjustments needed. The event was reclassified as an abnormal occurrence.

7 Designation of Apparent Cause of Occurrence: Failure in implementation of administrative controls.

Analysis of Occurrence: The valve was tested and met Technical Specifications isolation requirements following the maintenance work.

A secoua isolation valve in series is also fully operable and thus no undue hazard existed for this event.

9 Corrective Action:

Investigation fcllowing the PRC meeting of July 8, 1975 revealed additional problems with the use of MGP-b procedure and it was immediately invalidated. Four previous uses of the procedure were identified and reviewed and a determination made by PRC in meeting on July 16, 1975 that no problems exist due to those uses.

The subject of general procedures vs specific procedures for maintenance activities is being reviewed to determine if a satisfactory methoi can be developed. No use is made of such general procedures in other plant departments. All Maintenance Department general procedures are being reviewed to determine applicability and adequacy.

Supervisory personnel associated with the event have been verbally in-structed in requirements for procedure adherence and sign-off requirements.

The valve vill be retested again during the next cold shutdown following development of an adequate procedure to determine the margin of shut-off capability.

Failure Data: Reference event is A0-15-75 in letter of June 10, 1975 and use of the same procedure (MGP-4, Rev 0) appears to have contributed to the cause of that event when it was used for valve inspection and ad-justments in April of 197k.
