O Bristol Steel and Iron Works Inc.
Docket No. 99900371/80-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on November 18-20, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

Paragraph 10.6 of both the ASME accepted Quality Assurance Manual for ASME Section III Nuclear Power Plant Components and the customer accepted Quality Assurance Manual for Appendix B to 10 CFR 50 Nuclear Power Plant Safety Related Items state:
"All welding is performed by quali~
fied welders in accordance with one of the qualified Welding Procedure Specifications which is referenced on the applicable FCR or FCA and which is available in an area adjacent to the work and in QA Files for reference by the welders, welding operators, and the ANI."
Welding Procedure Specification, SMA-1.1-59, "no title, states:
"AWS Joint:
As detailed on approved shop drawings and qualifications."
Contrary to this requirement, AWS joint was not detailed on approved shop drawings F-0391/18-1 and 2, and F-0391/17-1, 2, and 3.
B.
Paragraph 7.6.12 of the TVA Design Specification, YCP-OS-1705-3506-01 states "The backside of all complete penetration double-welded joints made without the use of backing bar shall be arc gouged or back-chipped to sound and clean metal for width sufficient to allow effective mani-pulation of the electrodes prior to welding the second side."
Contrary to this requirement, neither the applicable drawings or the welding procedure SMA-1.1-59 required arc gouging or back-chipping to sound and clean metal.
