NOTICE OF VIOLATION As a result of the inspection conducted on March 12, 1981, and in accordance with the attached Interim Enforcement Policy, 45 FR 65754 (October 7,1980), the following violation was identified:

10 CFR 20.401(b) requires, in part, that each licensee maintain records showino the results of surveys required by 20.201(b).

Contrary to this requirement, a record of the radiation survey made on March 9, 1981, of the Unit 1 gas decay tank enclosere was not documented. The radiation survey was the basis for removing the enclosure from high radiation area status on that date.

This is a Severity Level VI violation (Supplement IVrF).

RESPONSE

A perscnnel error resulted in the failure to document the results of a radiation survey in the waste gas decay tank enclosure. On March 12, 1981, a radiation survey was performed and documented.

The survey determined that a major portion of the whole body could not be exposed to an exposure rate greater than 100 mr/hr.

To prevent recurrence of this violation, the individual that performed the survey and other Health Physics personnel were counselled on the requirement for properly documenting radiation surveys and the proper method to revise the High Radiation Door Inspection Report.

Full compliance has been achieved.
