ORGANIZATION:

The final responsibility for this Quality Assurance (QA) Program for the com-pliance with applicable Part 71 Requirements rests with Omaha Nondestructive and Metallurgical Testing, Inc. The Program is li=ited to the Receipt, Handling and Shipment of Radioactive Materials and does not include Design and Fabrica-tion. The QA Progra:2 is i=ple=ented using the following organization:

OUALITY ASSURANCE PRCCRAM:

The manage =ent of Omaha Nondestructive and Metallurgical Testing, Inc, estab-lished and implements this QA Program. It is required that training, prior to engage =ent, for all QA functions will be according to written procedures.

Any revisions to this QA Program will be according to written procedures and with management approval. The QA Program will ensure that all defined Quality Control (QC) procedures, engineering procedures and special provisions of the package design approval are satisfied. The QA Program will emphasize control of the characteristics of the package which is critical to safety.

The Radiation Safety Officer shall assure that all radioactive material ship-ping packages are designed and =anuf actured under a QA Program approved by the Nuclear Regulatory Cc= mission for all packages designed or f abricated af ter 1 January 1979. A copy of the manuf acturer's Certification of Compli-ance will satisfy this requirement.

LCCUMENT CCNTROL:

All documents related to a specific shipping package will be controlled in accordance with written procedures. Any revisions to written procedures will be performed according to =anagement approved written procedures.

l The Radiation Safety Officer shall insure that all QA functions are conducted in accordance with the latest applicable changes to those documents.

HANDLING, SICRAGE AND SHIPPING:

Written safety procedures concerning the handling, storage and shipping of packages for certain special form radioactive caterial,will be followed.

l Shipments will not be made unless all tests, certifications, acceptances l

and final inspections have been completed. Work instructions will be pro-vided for handling, storage and shipping operations.

Radiography personnel shall perform the critical handling, storage and ship-ping.

INSPECTION, TEST AND OPERATING STATUS:

Inspection, test and operating status of packages for certain special form radioactive caterials will be indicated and controlled by written procedures.

Status will be indicated by tag label, marking or log entry. Status of non-conforming parts or packages will be positively maintained by written proce-dures.

Radiography personnel shall perform the regulatory required inspections and tests in accordance with written procedures. The Radiation Safety Officer shall ensure that these functions are performed.

QUALITY ASSURANCE RECORDS:

Records of package approvals (including references and drawings), procure-ment, inspections, tests, operating logs, audit results, personnel training and qualifications and records of shipments will be maintained. Descriptions of equip =ent and written procedures will also be =aintained.

These records will be maintained in accordance with written procedures, identified and retrievable. The Radiation Safety Officer will maintain a list of these records and their storage location.

AUDITS:

Established schedule of audits of the QA Program will be performed by an audit team, having no responsibility in the activity being audited, using written check lists. The audits will be dependent on the safety signifi-cance of the activity being perfor=ed, but each activity will be audited at l

least once per year. Results of audits will be reported to management who will evaluate the reports and correct deficient areas. Audit reports will be maintained as part of the Quality Assurance records.
