Dear Mr. Goller:
This report is submitted in accordance with Section IX.H. 2.a. (2) of the Technical Specifications of License No. DPR-7. Noti-fication of this occurrence was made via telephone to the office of the Director of the Directorate of Regulatory Operations, Region V, on the day of the occurrence. This notification was confirmed by a letter to the Director on December 30, 1974. Report Number: DPR-7/74-6 Report Date: January 6, 1975 Occurrence Date: December 28, 1974 Facility: Humboldt Bay Power Plant, Unit No. 3, Eureka, California Identification of Occurrence: Following the reactor startup of Decem-ber 27, 1974, the reactor water conductivity exceeded the absolute limit specified in Table IV-2 of the Technical Specifications for a period of 30 minutes beyond the 24 hours permitted after a startup. Description of Occurrence: At 0421 hours on December 27, 1974, startup of the Unit following the scheduled 1974 refueling and maintenance out-age was completed and the Unit was paralleled with the system. After paralleling the unit, the reactor water conductivity was found to be 11 umho/cc. This exceeds the absolute limit of 5 umho/cc but is per-mitted by the Technical Specifications which state in Table IV-2 vosoaom P00R ORIGINAL
( r ) w Mr. Karl R. Goller 2 January 6, 1975 Note (3) " Conductivity absolute limit may be exceeded for the first 24 hours after any startup." Evaluation of the sampling results and cleanup rate during the period following startup indicated that the conductivity requirement would be met. However, the reactor water conductivity did not decrease as fast as originally predicted and, as a result, the conductivity was in excess of 5 umho/cc for 24-1/2 hours following the startup of the Unit. Designation of Apparent Cause of Occurrence: Condenser tube salt water leaks and below normal cleanup flow (due to loss of the cleanup pump which occurred on 12/23/74) contributed to a slower than normal reactor water cleanup rate following startup. Analvsis of Occurrence: Reactor water sampling results showed that all reactor water quality values except for conductivity and chloride ion concentration were below the operating limits specified in the Technical Specifications Table IV-2. The chloride ion concentration slightly ex-ceeded the operating limit (0.12 verses 0.10 ppm) for less than 19 hours after startup, but never exceeded the absolute limit (0.50 ppm). The high conductivity condition was due to the effect of soluble corrosion products rather than excessive chloride ion or abnormal pH, which could affect the integrity of materials in the primatv system. Corrective Action: The cleanup pump will be returned to service as parts to repair the pump are received. The conder ser tube leaks have been repaired.
