Dear Mr. Goller:

This report is submitted to provide additional in-formation concerning Abnormal Occurrence Report DPR-7/74-1.

The occurrence reported involved the poison injec-tion equalizing valve, which did not permit proper poison sample flow on March 27, 1974.

It was stated in the 74-1 report that the valve would be disassembled and inspected during our next refueling outage.

The inspection of the valve was completed on December 6,1974.

The valve body and piping were clean and in good condition with no evidence of rust, crystal or scale present.

However, excessive force was required to remove the stem from the top closure of the valve, indicating binding of the valve plug to the guide or valve packing to the stem.

The problem was traced to the valve packing restricting the stem motion.

The valve was serviced and reassembled using new teflon packing.

It has since performed satisfactorily.
