radwaste sump pump controls and the tankage area drain to sump valve were made. Operating, maintenance and testing practices associated with this system will be reviewed for possible changes. This is not a repetitive occurrence (LER 77-02).

CAUSE DESCRIPTION (cont)

(the concentrator drips drain to the sump).

The sump filled and should have overflowed, but the closed drain valve from the tankage area drain pit to the sump leaked due to a damaged seat

, (4" Clow F-3075). The waste then ran through the open drain pit to effluent canal valve and resulted in the release.
