4 APPENDIX A NOTICE OF VIOLATION South Carolina Electric and Gas Company License No. CPPR-94 V. C. Summer Based on the NRC inspection October 6,1980 through November 16, 1980, certain i

of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by 10 CFR 50, Appendix B, Criterion XVII, measures shall be established to assure that conditions adverse to quality such as failures malfunctions, deficiencies, deviations, defective material and equipment and nonconformances are promptly identified and corrected. In the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition. Section 17.2.16.2 of the FSAR states that corrective action controls are described in approved procedures and establish the method of identifying and controlling nonconforming conditions and other conditions adverse to quality.

Section 12.5.2 of the Operational QA Plan states in part that noncon-forming documents are evaluated by the start up supervisor to identify adverse quality conditions to determine the cause of the condition and recommend appropriate corrective action to preclude recurrence.

Contrary to the above, on November 11, 1980 corrective action controls were not described in procedures in that the Startup Supervisor was not required to identify adverse quality conditions to determine the cause of the condition and recommend appropriate corrective action to i

preclude recurrence.

Section 12.2.2 of the Operational QA Plan states in part that the Startup Supervisor reviews nonconformance and corrective action control procedures to assure that adequate controls are delineated to describe criteria for determining disposition in one of the following categories:

(1) accept as is (2) rework (3) repair or (4) reject.

Contrary to the above, on November 11, 1980, corrective action controls were not described in that procedures requiring the Startup Supervisor to review procedures delineating criteria for determining the dis-position of nonconformances did not exist.

This is an infraction.

As required by 10CFR50, Appendix B, Criterion V, implemented by FSAR Section I

17.2.5 " Activities affecting quality shall be prescribed by documented j

instructions procedures and drawings and shall be accomplished in j

accordance with these instructions procedures and drawings.

Start Up Manual Procedure SUM-B-13 " Start Up Field Reports" (SFR)

Section 4.4.2 states that a Category "SB" SFR shall - be limited to changes in working intent drawings.

Contrary to the above, SFR 1309, Recycle System SFR 1118 Locking Devices on High Head Safety Injection Throttle valves and SFR 2462, Teflon Tap on Charging /SI Pumps were classified as "SB" yet involved changes to other than working intent drawings.

Start Up Manual Procedure SUM-B-13 " Start Up Field Reports" (SFR) i Section 4.11 states that on SFR classified as "other" be limited to additional design information or clarification of s system engineering intent.

Nonconformance Notice Number 20, Feedwater Isolation Bypass Valves, states in part that the discs for valves XVT1678 A, B and C are accep-table (after rework) for reassembly /use only af ter minor diameter is verified to be 2.512

Contrary to the above, XVT 1678L was deemed acceptable with a minor.

I diameter of 2.570 inches.
