This office has received notification that your facility discharged oil or hazardous materials in harmful quantities in violation of Section 311 (b) (3) of the Federal Water Pollution Control Act Amend-ments of 1972, 33 U.S.C.1321 (b) (3) as referenced above. You are hereby requested to submit to EPA the following information:

Attachment Item o: Valve closed and locked. Storm drain system in area uns dammed up to prevent release of material to waterway. Line spread to neutrali:e acid. Contaminated ground to be turned over to a depth of 6"-10" to facilitate neutralization of acid with lime.

Contaminated pipes and cables to be flushed to remove personnel ha:ards. Area to be restoned to prevent personnel buards due to contact with acid or lime.

Kerry Harner - Plant Chemical Supervisor TMINS John S. Schork - Supervisor, Industrial Safety and Health, Unit II J. J. Barton - Manager, Site Operations TMINS Valve TF-WT-V3 was locked closed. A blank flange at the end of Item t:

the acid truck connection was installed to prevent 1meurrence of this type of event.

Certified letter was received at station on Saturday, January Item v:

31, 1981, but did not reach appropriate individuals for response until February 3, 1981. USEPA was called but could not be contacted on that date.

Mr. Harsch was contacted at 6810 hours0.0788 days <br />1.892 hours <br />0.0113 weeks <br />0.00259 months <br /> on February 4,1981 and an extension was granted undl February 10, 1981 to submit response.
