-Colt Industries Fairbanks Morse Engine Division Docket No. 99900300/80-02.

NOTICE OF DEVIATION Based on the results'of an NRC inspection conducted on September 15-18, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

Criterion V-of Appendix B to 10 CFR 50 states:

" Activities affecting quality shall'be prescribed by documented instructions, procedures, or' drawings, of a type appropriate 1to the circumstances and shall be. accomplished in accordance with these. instructions, procedures, 'or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

A.

Standard Practice No. 350.60, dated May, November 1978 and February 1980, contains.the following requirements:

1.

Paragraph 5.1 states, " Design changes resulting from design review or verification, analysis, or test, shall be initiat-J by the appro-priate Engineering Groups after concurrence of ot><,r affected depart-ments. These changes shall be subjected to design control commensurate with those applied to the original ~ design, including field changes."

2.

Paragraph 7.2.2, states in part, "All final calculations must be checked and signed by another analyst. The checking procedure shall be documented."

3.

Paragraph 7.2.4 states, "The Manager of the Analytical Department approves the ~eport, including affixing his Professional Engineer's r

stamp as applicable and forwards it to the Project Engineer with all the supporting documents."

Contrary to the above:

Final calculations for the seismic analysis, dated December 19, a.

1978, for the North Anna diesel generator battery racks, had-not been signed by another analyst.

.The Manager of the Analytical Department had not approved Revision 1, dated December.22, 1978, of the Seismic Report for tha North Anna diesel generator battery racks.

See Details Section, paragraph B.3.b.

B.

Fairbanks Morse Engine Division (FMED)-documented the problem of wrapped.

mounting pads on Limerick heat exchangers on Bechtel's Supplier Deviation Disposition Request No. M71-35.

FMED's disposition of the deviation, states in part, " Wedges and shims will be tack welded in place to lock them per-manently in position.

Colt welding procedure. 16:23:02, previously approved by Bechtel, will be used."

Bechtel concurred with the disposition.

Contrary to the above, welding procedure No. 16:23:02 had not been used-to tack weld the_ wedges and shims to heat exchanger, unit number 6.

Welding Procedure No. 16:05:03 had been used to accomplish the welding.
