Dear !!r. O'Reilly:

On 11/21/80, a substantial safety hazard as defined by 10CFR21 was reported to Mr. John K. Rausch at the NRC Region II office. The item 4-involved the omission of an interlocking contact in the. swing component cooling pump that prevented it from starting on a load sequencer signal when it is lined up as the primary pump. This was an error in the design as submitted,by the architect engineer. Details are given on the attach-

_ ment.

The item is being reported under our 10CFR50.55(e) procedure. This is considered a final report on the matter.

Nature of Defect, Substantial Safety Hazard Created, and Evaluation An interlock contact was left out of the control circuit of the " swing" pump that would prevent it from starting on a load sequencer signal when it was lined up as the A or B primary pump. This could have resulted in loss of cooling water to the RHR and other safety related systems.

This was identifiad by startup field report 2577 dated 5/31/80.

Corrective Action The interlock has been inscalled in the circuit. The architect engineer has been notified, but feels that this is an isolated incident and no procedure or design criteria revisions are necessary.
