RWP 2365 was in effect for the purpose of control of the reactor floor during the present spent fuel shipping program. Prior to issuance of RWP 2365, RWPs for this area called for use of canvas shoe covers, coveralls, and canvas gloves to be worn, with an asterisk referring to a footnote that stated " Protective clothing to be worn at the discretion of the HP tech." RWP 2365 had the asterisk placed by the protective clothing requirements, but the footnote that, " Protective clothing was to be worn at the discretion of the HP tech," was inadvertently omitted from the doc.ument. Personnel working the job were aware of the asterisk but f ailed to notice that the footnote it referred to had been omitted. They were however, under verbal control of the HP tech as to protective clothing, as stated on each of the previous RWPs issued for this work. Immediate corrective action was taken in the form of admonishing the spent fuel shipping crew for failure to read the RUP in its entirety and noting that it was technically incorrect. Follow-up action of ree=phasizing the RWP requirements for entry into an RWP area and the importance of reading the RWP each time of entry was completed with each maintenance department employee. This was done as a direct result of the Fort St. Vrain Review Panel directive that required all plant supervisors to cover RWP requirements with their employees. While there was no evidence of flagrant violation of procedures, it is recognized that each condition of any control document must be read understood, and adherred to and that verbal instructions must be limited to only situations where i= mediate action must be taken to respond to changing or emergency conditions that affect personnel, the public, or plant safety. In order to prevent recurrence of these problems, you are directec to re empnasice to your employees, the following: The RWP is the governing document. It is mandatory that RWP's be read anc followed. RWP instructions are to be followed ratner than Health Physics Technician verbal instructions, except in cases of emergency, wnere a Health Physics Tecnnician may cirect personnel to vacate the area. All personnel have receivec Health Physics t-aining anc are not to rely on tne Health Physics Deoartment for instructions. Work may be cone in an area without an RWP under the verbal directions of a Health Physics tecnnician. Even though departments are understaffed and work icacs are neavy, personnel must take the time to be tnorougn, to do the job

right, and to do the job safely No corners ar7 to be cut on safety.
