1980 References 1 and 2 provided Consumers Power Ccnpany's response to IE Bulletin 79-02.
Reference 3 stated that we had determined that further actions were needed to provide corrections to the response provided by Reference 2.
Further investigation discovered specific cases where a statement in the attachment to Reference 1 was incorrect due to the specificatien requirements having been misapplied in the actual installation. The attachment to Reference h provided a summary of the actions being taken to obtain the required infomation and the status of actions in progress as of June,1980. This letter provides a current status of the actions being taken.
The attachment to Reference h identified three (3) primary areas of error in the response to Bulletin 79-02 as follovs:
(1) anchor bolt use prohibition, (2) deter =ination of proper embed =ent depth, and (3) demonstration of achievement of required factor of safety. The following is a status of the actions which have occurred and are in process since the Reference h sulcittal.
1.
Anchor Bolt Use Prohibitien Meetings have been conducted between Bechtel Project Engineering and the pipe support design agency to describe and identify the extent of the proble=. The folleving are specific accceplishments:
A.
Pro,)ect Engineering is re-evaluating the criteria for use of drilled-in anchor bolts. The specification currently prohibits the use of anchor bolts in certain locations based on cyclic loads.

Design criteria for base plate analysis which considers the effects of the use of grouted anchor bolts, rather than drilled-in anchor bolts, is in the final coordination phase between Bechtel Project Engineering and the pipe support design agency.
2.
Determination of Proper Dntedment Derth The following actions are in progress:
A.
Project Engineering is coordinating with Bechtel Material and Quality Services (M&QS) to establish acceptability of the UT procedure actually used to deter =ine anchor bolt depth. The procedure used had prior Engineering approval for applications which did not specifically include establishing length of anchor bolts.
B.
Engineering is currently evaluating the failure to perform the inspections s iainst a controlled random sample, using numbered bolts and randcm ic.cber tables.
If Engineering is unable to justify the ss=ple selections, inspection of embedment depth vill be incorporated into the specification requirement for a total re-inspection of stud-type bolts described in paragraph h below.

Demonstration of Achievement of Required Factor of Safety Project Engineering has completed the initial draft of a specification which addresses anchor bolt testing to establish the relationship between torque and tension of the bolt in concrete and its ultimate pull-out strength in several grades of concrete. An initial draft specification covering the testing of bolts to establish the a=ount of relaxation in the tension of an anchor bolt over a period of time has also been completed.
Release of these specifications and initiation of the inspection and test program is imminent, b.
MCAR Status Bechtel's Management Corrective Action Reports numbers MCAR-31 and MCAR-3h remain open and vill remain open until all required actions for them are completed and verified.
A.
A specification is in the final coordination process which will cover the reinspection required in response to MCAR-31 on installed expansion stud-type anchors and vill provide ca:plete details and instructions for performing these inspections and providing infor-mat '.on to engineering for analysis. Analysis of the data vill be contingent on completion of the inspections required by this specification and completion of the evaluations described in paragraph 2, above.

The corrective action progress of the response to MCAR-3h and 50 55(e) item on drop-in anchors is continuing. An inspection specification has been issued and approximately 25 percent of the "Q" drop-in anchors installed by Bechtel have been re-inspected to t
this new specification. The data resulting from that inspection are being provided to Engineering and analysis of the data is continuing.
5 Reportability Reviev j'
indicates that significant numbers of anchors do not meet the design A review for reportability will be performed if the reinspection data requirements.
Either a final response or a status of the actions being taken to provide the response will be transmitted to you on or before January 30, 1981.
