Description of Particular Event or Circumstance:

i Vnile performing the control rod interchange during tiie present scheduled outage, it was observed that a bolt was lying on top of the lower core support plate'.

Observations made with a T.V.

camera through 40 feet of water indicated that there were two j

additional bolts lying loose. An examination of the bolts indi-cate that they are control rod shroud tie down bolts..

Preliminary determination identifies two shroud' tubes displaced.

There are a total of 32 shroud tubes, 8 for shim rods and 24 for moveable control rods. All T.V. monitoring has been recorded on tape.

Action by Licensee:

The licensee has contacted Westinghouse to assist in conduct-ing a detailed internal inspection which includes removal of the lower core barrel.

The core was completely unloaded on November. 11, 1972. This includes all fuel, control rods and sources.

The Plant Operating Review Committee has met and reviewed the j

problem.

The Nuclear Audit and Review Committee has'been informed and corporate officers have been notified and have been on site.

The plant will remain shutdown for an undetermined time.

A written report will be submitted to the AEC within 30 days in accordance with 10 CFR 20.405.
