Event Descriotion:

b Ouring normal coastdown operation, an operator noted that the 2

No.1 diesel generator's control circuit supervisory lamp was out. In-ij i

vestigation revealed that a circuit fuse was blown.

Immediately after d

the incident the No. 2 and No. 3 diesel generators were started and test run thus verifying their operability.

A similar incident occurred to y

No. 3 diesel generator and was reported as Abnormal Occurrence No.

U 50-29/75-5.

This incident is a violation of Technical Specification, Section E.4.d.

(Yankee-Rowe Abnormal Occurrence No. 75-09)

,j Cause Descriotion:

t The General Motors, Model 7162-7000, diesel engine's control circuit blown fuse resulted from the local supervisory lamp fusing into the lamp socket due to overheating.

Because of the repetitiveness of this incident, plans are to investigate replacing the local supervisory: lamps i

and sockets of all three diesel generators with those of a different design and/or voltage rating.

Dear Mr. O'Reilly:

Pursuant to the requirements of the Technical Specifications Section E.4.d, a failure of an engineered safety feature that causes or threatens to cause the feature or system to be incapable of performing its intended function is reported as Abnormal Occurrence 50-29/75-09.

At 0720 on 9/6/75, with the plant operating at normal steady state power, a blown fuse in the'No.1 Emergency Diesel Generator DC starting circuit resulted from the overheating and subsequent fusing of a starting circuit supervisory lamp.

Ir=ediately after the incident the 110. 2 and No. 3 Emergency Diesel Generators were started and test run for five minutes thus veri-fying their operability.

At 1006 the same day a repair was completed and the No.1 Emergency Diesel Generator was test run and declared fully operational.

A formal 10 day report will follow.

Very truly yours,
