Dear Mr. Engelken:

This is a report of an evaluated defect as defined in 10 CFR Part 21.

A verbal report has been made by telephone to you on August 13, 1980.

The following information is provided by item, numbere'd according to the outline of required.information in the regulation:

(ii)

The component supplied, which has within it electronic parts under excessive stressxis identified as follows:

Part Number: 0281-0410-02 Part flame:

Area Monitor, Countrate Assembly, RP-2C (iii)

The firm supplying the component described in (ii) 1:.:

General Atomic Company Electronic Systems Division P.O. Box 81608 San Diego, Ca 92138 (iv) flature of Defect - Th.e mcnitor assembly identified above has four resistors that are over-stressed when the internal high voltage supply is adjusted for voltages above 800 vdc. The i

normal voltage for the Class lE application is about 850 vdc.

At lower voltages, the stress diminishes and fewer resistors are over-stressed.

(v)

Date of Infonnation - The defect was determined on 07/30/80 after examining a unit which exhibited a reduced high voltage output.

(vi)

The following are the only locations to which Class lE qualified components have been supplied.

The respective customers have been advised of the forthcoming change.

Corrective Action - The following actions are or will be accomplished:

1.

All purchasers of Class 1E qualified units were notified of the potential problem on 8/01/80.

2.

The design will be modified to reduce individual part stresses to less than the parts design limits.

This task will be completed by 8/15/80.

3.

All purchasers will be advised of the needed modifications and will be offered rework service or a supply of parts for their local rework task.

It is expected the parts can be obtained soon enough to permit implementation of this task by 9/02/80.

Purchasers of similar unqualified versions of this product will also be offered the same service or parts.

4.

All future shipments of this product will incorporate the changes resulting from step 2.

(viii) Advice to Purchasers - The two purchasers who have taken delivery have been advised (see (vii) item 1).

These same pu/ chasers, along with purchasers of similar but non-Class lE units will be provided information about the defect and will be provided rework service or parts and instructions.
