All EPA's used for Non-class 1E circuits with f 6 AWG, #2 AWG,

  1. 4/0 AUG, and 350 MCM terminations.

The 18 non-inspected EPA's contain like terminations as those of Group These EPA's are used for Non-class 1E circuits and were to be included 3.

indicated the need.

"'he inspection consisted of a visual exami2ation and a " pull test"

crimped.

All testing and rework was vitnessed by Bechtel Quality Control personnel-in addition to partial surveillance by Bechtel Electrical Field Engineering and Consumers Power Company Quality Assurance personnel.

The results of the inspection are:

A)

-74 EPA's were inspected 17,714 terminations total.

B)

No unsatisfactory terminations were found in 54 EPA's - 12,631

' terminations total.

C)

Unsatisfactory terminations were found in 20 EPA's - 93 unsatis-factory terminations out of 5,083 terminations total.

Based on the total terminations of "C", the percent of unsatisfactory terminations is 1.83%.

However, based on the total terminations'of all the inspected EPA's.("A"), the percent of unsatisfactory termidations is 0.5252.

No improper terminations were found in the EPA's of Group 3, therefore,

-l it was determined :that no additional inspection would be required.for the 18 EPA's that contained like terminations.

~ ~" terminations were reworked and passed further inspections and pull tests.

In. addition, all terminations were' checked for:

1)

Proper type of lug.

2)

. Proper. lug indentation.

3)

Tightness to terminal blocks.

i As process corrective action the supplier has initiated a 100% inspection s

program consisting of, _ visual inspection, pull testing, and continuity testing for all terminations on all current and future nuclear projects 1

and has established a special training program for all nuclear production personnel. This program has been initiated as of April 24,1979 and therefore should prevent iteoccurrence for~any potential add-on order:

H Safety i.mplication If this condition had gone undetected, the ability of the EPA to perform its intended safety function may have been compromised. Therefore, this j

condition was determined to be reportable.
