Initial. notification of the subject deficiencies was made'to NRC-OIE, Region II, Inspector R. W. Wright on May '5,1980, for NCR-10 and May 13,-
1980, for. NCR's HTN CEE. 80-01 anFPBN QE3 80-01. : Cur-final response was submitted on December-18, 1980.
In order to meet ~ the.10-day -reporting e commitment made in the March 20, 1981, TVA-NRC meeting,.we are. enclosing a revised final reportion the subject'deficiencieis.' This revision.was 1
necessitated' by the faci, that-TVA has: terminated its contract with the supplying vendor (Atlas Machine and Iron Works, Gainesville, Virginia).

On May 5; 1980, IVA.. informed NRC-OIE Region' II. Inspector, _ R. W.. Wright,.of a potentially reportable condition 'under.10CFR50.55(e) regarding 'a steam tunnel embedment ' stiffener plate. weld.- This st'ructure w'as : manufactured.by Atlas' Machine and. Iron Works'.( Atlas), Gainesville, Virginia. - On May. 13, 1.980, TVA informed R. W. Wright;of a potentially reportable condition under 10CFR50.5.5(e) regarding a breakdown in the Atlas-QA~ program.'- This is our revised final report on these items.

When_ Atlas presented ' the.'Hartsville unit A.1 steam tunnel.e=bedment to TVA for shipment, it was rejected because some critical di=ensions were not within allowable' tolerances, causing severe mistatch between subsections.~ - To repair this condition,.the.asse=bly had to.be cut into segments for realignment. When this was done, a TVA inspector noticed a backing bar which was not flush to-a stiffener plate.
Upon removal'ef the backing bar, it was observed.that this sea had not beeh co=pletely welded. The end stif.fener plate in upper column 1R was welded on three sides only. 'This instance of a deficient weld Js docu=ented on NCE-10.
This condition indicates that the~ Atlas _ inspection progra: is deficient.
On April 29. thr.ough May 2,1980', TVA' conducted 'an audit of-Atlas. 'This audit t
revealed 16 deficiencies in the implementation of the Atlas. QA program, nine of which are considered:significant. These deficiencies' involve document control, material and process. control,-OA records, and training, and are 'ocumented on-d NCR's HTNQEB8001-and PBNQEB8001.
' Safety Implications Were it not 'for the fact. that some' joints'were out of. tolerance, the missing weld may net have been discovere'. Consequently, when this deficiency-was d
found, TVA had no assurance that other similar' deficiencies did not ex_ist.
Missing or ina'dequate welds within the structure could degrade the structural integrity of the embedment which could adversely affect plant safety.
.Cerrective Action
'The deficient weld on the. stiffener plate (NCR-10) was made and inspected per QA requirments. TVA had two resident inspectors at Atlas to supervise inspection e
and repair of the*steac tunnel,embedment and =enitor other contracts.
A contract engineer and a QA evaluator were also sent td Atlas to advise and aid in contractual and QA program difficulties.

Atlas hired a' Q Iconsultant to, aid in the ' development of appropriate QA procedures. Atlas also, changed QA managers and increased the inspection staff.
After th~ese, changes.were made, TVA c~onditionally' accepted the Atlas QA program and verified that implementation was considerably improved in'an audit which was conducted on November 3 through 7,1980. During this audit, all previcus deficiencies were evaluated and all except one regarding implementation of the shop traveler ' system.were closed.
No.new items were fou.nd during this audit.
On December 18,.1980, TVA reported' that the Atlas QA ' program was b'eing satisfactorily implemented but.that another audit was ' planned, to monitor the
. quality of work being performed by Atlas. However,~further.auditin6 and inspbetion by TVA' revealed recurring documentation problems in the assembly and manufacture of material being performed by Atlas and in the general implementation of the Atlas.Qf program. These repeated deficiencies were greatly impacting the construct. ion sched' le for Hartsville unit A1,to an extent u
that was unacceptable because almost all work was having to be redone.
Conseque'ntly,.TVA has terminated its contract with' Atlas and is ne6otiating with other vendors to perform'this work.
