Inspection on January 7 and 8, 1981 (Report Nos. 50-373/81-01; 50-374/81-01)

Areas Inspected: Rip rap on spillway; followup unresolved items. Suppression

pool modifications; safety relief valves modification and installation. The

inspection involved a total of 12 inspector-hours on site by one NRC inspector.

Results: No items of noncompliance or deviations were identified during this

inspection.

Licensee Action on Previously Reported-Items

(Closed) Unresolved Item (50-373/80-42-01): Compatability of Fire Code CT

gypsum material with cable jackets. Collyer Cable Company laboratory report

78-18 of cable jacket material coated with firecode CT gypsum and air oven

tested at 90 C for 100 days, shows no deleterious effects; neither chemically

nor physically to cable jacket. Kerit Company letter to U.S. Gypsum dated

May 2, 1980, indicated that gypsum will have no degrading effect on their

cable. Okonite Company letter to CECO SNED dated September 18, 1978, states

that, based on their investigation that Firecode CT gypsum would have no

deleterious effect on Okonite cables. Since cables at La Salle are jacketed

with hypalon, neoprene or kerit, and these materials have been reviewed for

-

compatability with gypsum materials this item is resolved.

(Closed) Unresolved Item (50-373/80-42-02): Ability of gypsum fire stops to

maintain control room integrity. Cracks noted during ASTM E-119 fire tests

did not extend-through the penetration. Vapors noted during the fire test

were determined to be steam from the gypsum. The control room is pressurized

to prevent the entrance of noxias gases and a breathing air manifold is in-

stalled to permit the operators to perform their functions in the presence of

smoke. No chlorine gas is used for water treatment at La Salle since all algae

treatment of heat exchangers_is accomplished by using calcium hypochlarite.

(Open) Item reported pursuant to 10 CFR 50.55(e) (50-374-80-xx-02): Cracked

insulation on wiring external _to containment penetrations. All Amphenol

pentrations for instrument and control cables returned to manufacturer's

facility. Test procedures have been developed and approved by owners.

Procedure requires insulation resistance test be performed while the

penetration assembly is immersed in an agueous electrolytic solution.

This item remains open pending completion of penetration testing and

repair.

Program or Functional Areas Inspected

Safety Relief Valves

The reworked safety relief valves from Unit 2 have been installed on Unit 1.

The inspector examined documentation including Form NV-1 Code Data Reports

for 19 reworked safety relief valves.

The NV-1 forms indicate that disc

inserts, nozzles, bonnet stud nuts, adjusting bolts, and the thrust bearing

adapters have been replaced. The bodies, spring washers, bonnets, spindle

assemblies were remachined and an adjusting bolt button assembly added to

each valve.

Instrumentation

The inspector examined documentation pertaining to instruments purchased

by the site mechanical contractor. Documentation for flow orifaces included

purchase orders, certified material test reports, data sheets and flow curves.

Rosemount Model 1152 transmitter documentation includes test reports by Wyle

Laboratories of seismic tests per IEEE-344, pressure tests required by ASME

Section III, and environmental testing performed in accordance with the

requirements of IEEE-323.

Suppression Pool Modifications

The structural contractor, CB&I has completed the installations of additional

bract.* in the suppression pools in both units. Painting contractor is pre-

sently sandblasting and painting carbon steel added during the modifications.

- The inspector examined reports of three audits and seven surveillances conducted

on' activities performed by CB&I during modifications to the suppression pools.

Exit Interview

The inspector met with licensee representatives (denoted under Persons

Contacted) at the conclusion of'the inspection on January 8, 1981. The

inspector summarized the scope and findings of the inspe'ction. The licensee
