Beaver Valley Power Station - Unit No. 2 Docket No. 50-412 Significant Deficiency 581-01 Use of an Unauthorized Document as the Basis for Quality Control Acceptance Gentlemen:

Mr. H.'A. Van Wassen orally reported a potential 10CFR50.55 (e) reportable deficiency to Mr. R. Feil, NRC, Region I, on February 18, 1981. The potential Significant Deficiency, SDR $81-01, . concerned the use of a Request for Information, RI-e0517-DC, as'the basis of Attribute S-303 in our Inspection Plan 6.2.3, allowing Grade 60 reinforcing bars to be substituted

-for Grade 40 reinforcing bars. The use of the unauthorized document was effective between December 1978 and November 1979.

The original incident related to the acceptance _~of six #11 l

' Grade 60 dowels in lieu of six #11 Grade 40 dowels-in the lower pump' cubicle, Drawing RC-48 Series.. A Nonconformance and Disposition' Report, #1522, dated February 17, 1981, was raised L for Engineering evaluation.

Resulting from this' potential Significant Deficiency, a detailed

-review of existing. Inspection Plans has been initiated. This

- review has been. applied to all_. disciplines. To date we have reviewed 27 Inspection. Plans out of the 55 applicable Inspection-Plans involved. . The_results of-this review, to date, has shown 95  %

  • 4 Mr. Boyce.H. Grier Page 2 March 16, 1981 that the application of an unauthorized document to Attribute S-303 for an accep ance standard can be considered an isolated incident.

The review dLd indicate the utilization of similar documents in approximately eight instances. We consider their application was redundant, as approved Specifications / Drawings contained similar instructions.

Based on the results of this review, we believe thTt the problem discovered was the result of an inadvertent error during the Inspection Plan review cycle and is not indicative of a systematic or significant breakdown of our Quality Control Program.

The review establishing the source document for inspection attributes is continuing, and is scheduled to be completed prior to April 17, 1981.

The corrective action will be a revision to our Quality Control Procedure SQC-4.1, " Preparation of Inspection Plans", to include an improved definition of the documents to be used as a basis for_ attributes and the recording of the attribute source document in _ the historical file of our Inspection Plan Manual.

This action will be completed by March 31, 1981.

The original incident, described by Nonconformance and

- Disposition Report #1522, has been evaluated by our Engineers and is not considered to have a safety implication. The potential reportable deficiency has been determined as not to be reportable under 10CFR50.55(e).
