Circular required that Licensees take action to conduct a review of plans or programs which provide positive assurance that members of reactor operating and support staff are, in fact, complying with the i

safety procedures in effect and that they are aware of safety related incidents that have occurred at their facility as well as other similar facilities.

The Northeast Nuclear Energy Co. review has been completed. A response is included below each area of concern.

-1. Describe your Program for periodic shift and operator training whereby incidents which occur at your facility as well as at other licensed re-actors, including all significant personnel errors, will be reviewed with the objective of identifying "the lessons to be learned."

RESPONSE

The retraining program for licensed ~ personnel contains programs which provide information and/or instructions relating to incidents at other facilities.

The training coordinator reviews all licensee event reports for other units and l

-forwards pertinent reports to the operations supervisor for routing to licensed personnel and inclusion in the document acknowledge book.

For.. major. or. generic incidents formal instruction is provided as' part of' the retraining lecture program.

For incidents, which occur on Millstone units, plant incident reports are l

originated and filed in the respective control room. All incidents are reviewed by the operations supervisor. Required information / instructions are then pro-vided at regularly scheduled meetings for department supervisors, are spelled out in the night order book for operators, or are included as part of the retraining lecture program.

Consider Procedures routinely implemented by knowledgeable individuals to qualitatively assess the performance of the operating and support staff in such areas as adherence to operating procedures, use of systems checklists, and imple-mentation of component and system tagouts.

This should include review of the degree to which operating procedures, tagout procedures, and checklists require signoff, i.e., signature or initials to verify proper completion and to identify the respon-sible personnel.

RESPONSE

The proper application of a procedure for performance of a specific task is monitored by the cognizant department supervisor.

Examples are routine reviews of completed surveillance data, preventative maintenance data, procedure sequence checklists, and valve lineup sheets. Additionally, department supervisors routinely review all plant incident reports, work control authorization documents, ( job orders, maintenance requests ),

and all log books.

-tagout is accomplished, that prerequisites are met, and that procedures are followed.

The retraining program for licensed personnel also includes provisions for evaluation of operations personnel during abnormal or transient conditions.

A significant part of the evaluation is observation of compliance with procedures.

Additionally the Quality Control department performs routine audits of system tagging, lifted wires / jumper installation, surveillance performance, and system retest performance. These Audits document deficient items, which are dispositioned _in.a timely _ manner. This program is administered by Station Procedures 18.01 and 16.01.

Consider Procedures for random backshift and weekend visits by management and supervision to the facilities, to monitor and assess operations including crew manning and performance, equipment status and plant conditions.

RESPONSE

The nature of-plant operations, routinely provides the opportunity for plant management, department supervisors, and duty officers to ob' serve the adequacy'of crew manning, crew performance, equipment status and plant conditions.

This routine involvement of management and supervision on a backshift and weekend

' basis,-is sufficent for providing the necessary assurances that operations are conducted in an appropriate manner and precludes the need for formal procedures specifying random visits.
