FOR TE UA.IVLRSITY OF VIRGIIIIA Pl' ACTOR FACILITY A. General

1. This requalification program applies to licensed individuals of both the University of Virginia Research Reactor (UVAR) and the low power training reactor (CAVALIER), located at the Reactor Facility of the University of Virginia.
2. The operations =anager of the reactor facility shall be respcusible for ad=inistering the re-qualification progrs=.

1. Each licensed individual vill be required to ec=plete all check lists and perfom a start-up of the reactor at least once every three =enths and at least 10 ti=es during the period of his license. All individuals who are licensed for both the UVAR and the CAVALIER vill be required to com-plete at least five start-ups on each facility during the period of his license. If a licensed individual fails to perform these functions for a period of four =onths er longer he vill be re=oved from licensing activities and be required to take either a written or oral examination similar in scope to the A.I:..C. exa= and an operating test under the direction of a qualified Serior Operator before being reinstated as a .

qualified licensed operator. A grade of 70, or Greater vill be considered passing. The results of these exams shall be documented.

2. Each licensed individual vill be required to attend a series of lectures at least once each year. The lectures will cover variou:, aspeci.:t of the reactor operation including theory and principles of operation, plant operatin6 characteristics, plant instrumentation and control systems, plant protection syste=s, safety systems, normal, abnormal and emergency operating procedures, radiation control and safety, and technical speci-fications.

Once a year a written exam vill be given to each licensed individual to detemine his continuing knowledge of the facilities operations. If the results of these exams disclose deficiencies in certain areas, additional lectures may be given to cover the subject in more detail.

A grade of 70'4 or better vill be considered passing. The individual administering and gradin6 these exa=s vill be exempt from taking the exam.

3. Standard operating procedures and abnormal emergency procedures will be reviewed annually with licensed individuals as part of a lecture series.

A1111 censed individuals will be informed of system chances or pro-cedural changes by a memo issued by the operations manager. An up-to-date copy of the standard operat'.n6 procedures vill be kept in the control room of each reactor at all times.

h. A personal file shall be kept on each licensed individual. The file shall contain records on chech-lists and start-ups performed by the

individual, his attendance at lectures and tre content and results of vritten exams.

The operations =anager vill take periodie independent evaluations of each individual by observing the ccepletion of check lists and start-ups and control canipulations by each operator. Co m ents on these ot~frvations will be noted in the individual's file. If an operator is found to be deficient in some area of operation this vill be docu=ented and the appropriate action taken to correct the deficiency will be noted.
