NOTICE OF DEVIATION 3ased on the results of an NRC inspection conducted on September 22-26, 1980, it appears that certain of your activities were not conducted in accordance 3
with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings'of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriMe quantitative or qualitative acceptance criteria for determining that imp 6 Lant activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
A.
QC Procedure 14.1, paragraph 2.2.2.1, states in part, " Quality Appraisal personnel shall complete an applicable Quality Status tag whenever a rejec-tion notice is prepared on material physically located at the Chattanooga Operations.
"The applicable red or orange adhesive tag shall be attached by the originator directly to the item and a copy shall be attached to the appropriate traveler at the applicable location."
Contrary to the above, an applicable orange Quality Status tag was not attached to the appropriate traveler for Contract No. 11873 Job No. 743121-090, although a rejection notice had been prepared for the item.
B.
QC Procedure 14.1, paragraph 2.2.2.3, states, "Upon receipt of a rejection notice with an attached orange status tag, the cognizant quality engineer and process engineer shall review the nonconformance to determine the maximum point to which labrication may continue prior to clearing the discrepant condition. The applicable traveler (both material and file l
copy) shall be annotated that work is not to proceed until the rejection notice is completed."
Contrary to the above, the applicable traveler (material and file copy) had not been annotated that work was not to proceed until the rejection notice No. 3454 had been completed for Contract No. 12873, Job No. 753121-170.
