INSERVICE INSPECTION - EXAMINATION

SUMMARY

PRAIRIE ISLAND NUCLEAR GENERATING PLANT - UNIT 1 September 2 to October 13, 1980

1.0 INTRODUCTION

This report is a summary of the examinations performed during the fif th inservice inspection at the Prairie Island Nuclear Generating

Plant - Unit 1. This was the last inservice inspection conducted for inspection period two. The examinations were performed during the plant's fifth refueling outage from September 2 to October 13, 1980. Prairie Island - Unit 1 began commercial operation on Decem-b er 16, 19 73.

This report identifies the components examined, the examination methods used, the examination number; and, summarizes the examina-tion results of each of the following areas:

1. Pressure retaining components and supports of the reactor cool-ant and associated systems classified as ASME Class 1 and ASME Class 2.
2. Reactor pressure vessel examinations; including examination of nozzle bores for underclad cracking.
3. Bolting of seismic supports classified as ASME Class 1.

2.0 ASME CLASS 1 and 2 COMPONENTS 2.1 Examination Plan The examination plan focused on the pressure-retaining components and their supports of the reactor coolant and associated auxiliary systems classified as ASME Class 1 and Class 2.

The esamination plan was based on the examination requirements of the ASME Boiler and Pressure Vessel Code Section XI, 1974 Edition through and including the Summer 1975 Addenda, and com-plied with Prairie Island's Technical Specification, Section TS 4.2. The examination is in accordance with the program sub-l mitted to the United States Nuclear Regulatory Commission on February 1,1978 entitled, "ASME Code Section XI Inservice

, Inspection and Testing Program and Information Required for NRC Review of Requests for Relief from ASME Code Section XI Requirements . "

2.2 EXAMINATION METHODS Ultrasenic examination methods and techniques were used to per-form the volumetric examinations. The ultrasonic test systems consisted of an ultrasonic digital analog tester and a two channel strip chart recorder. One channel of the recorder was calibrated to reflect ultrasonic screen height (amplitude) and Page 1 of 6

the second channel was calibrated to indicate metal path (range) to the reflector. This approach to the examination gives a per-manent record to the extent possible.

Radiographic examinations were usad to further evaluate the ultra-sonic examinations as the need arose. The radiographic examina-tions were performed using a double wall exposure, single wall viewing technique. An Iridium 192 source was used in conjunction with multiple Kodak "M" film. In addition, the radiographic examinations were performed to the 2T penetrameter sensitivity level; and, where geometry permitted, for 'a distance equal to two wall thickness on each side of the veld.

Liquid penetrant or magnetic particle examination methods were used to perform the surface examinations. The liquid penetrant examinations were performed using color contrast-solvent removable materials. Magnetic particle examinations were performed using either a yoke with dry powder or an A-C L-10 coil with fluores-cent prepared bath.

All visual examinations were aided when necessary with artifi-cial lighting and verified for adequacy with an 18% neutral grey esrd with a 1/32 inch black line.

2.3 Examination Procedures The ultrasonic examination procedure for pipe welds complied with the requirements of Appendix III of ASME Section XI that was issued in the Winter 1975 Addenda. The radiographic ex-amination procedure complied with the requirements of ASME Section III,1974 Edition through and including the Winter 1976 Addenda; a field change was issued to this procedure to require 2T penetrameter sensitivity in lieu of the 4T essen-tial hole referenced by the procedure. All other examination procedures complied with the requirements of the 1974 Edition through and including the Summer 1975 Addenda of ASME Section XI. A listing of the procedures used for the examinations is I shown in Table III of Appendix E.

2.4 Equipment and Materials All equipment and expendable materials used in the examina-i tions are listed by either serial number of type along with I their respective calibration date or batch number in Table IV l, of Appendix E.

Thc ultrasonic calibration standards used in the examination are listed in Table II of Appendix E. These standards are owned and maintained by NSP at the plant site.

2.5 Personnel Northern States Power Company contracted Lambert, MacGill, Thomas, Inc. and Peabody Testing Services to perform the examinations. Hartford Steam Boiler Inspection and Insurance Company, representing ANI, provided the Authorized Inspection.

All personnel involved in the performance or evaluation of examinations are listed along with their title, organization, and ASNT Level of certification in Table I of Appendix E.

Certifications for examination personnel are maintained on file by Northern States Power Company.

2.6 Evaluation Any indications disclosed in the examinations were evaluated by the examiner at that time, in accordance with the rules of the procedure and ASME Section XI.

The ultrasonic examiner was aided in his evaluation by a cali-bration performed on a standard reference before each day's examination, checked before and after each individual examina-tion, and at intervals not exceeding four (4) hours. In addi-tion, the ultrasonic data was recorded on strip charts which stre made a part of the inspection report, and permitted further essluation.

2.7 Exatiination Reports and Documentation All examination reports and documentation are maintained on file by Northern States Power Company. Table I of Appendices A, B and D identifies the examination report number (s) for each item examined. Fbny of the items identify more than one exam-ination report because of the different types of efaminations performed on the item.

Table I of Appendix A, B, and D su=marizes all the examinations that have been performed to date and identifies the amount that will be examined in the future to complete the ten year examina-tion requirements. For retrieval purposes, the prefix of the inspection report number corresponds with the year that the insp-ection was performed. The examination report numbers for this outage are prefixed with "80A ".

Table II of Appendix A, B, and D compares the baseline examination results with the results obtained during this examination. Table III af Appendix A, B, and D identifies the isometric drawings that were used for the examinations. The personnel, ultrasonic calibra-tion blocks, procedures, equipment and materials that were used for the inspection are identified in the tables of Appendix E. Appendix F contains the Form NIS-1, entitled " Owners' Data Report for In-service Inspections".

2.8 Summarv of Results The evaluation of all the results from the inservice examinations indicated that the integrity of these systems has been maintained.

The anomalies detected appeared to be of fabrication and install-ation origin, and not from service.

These anomalies were:

The loose bolt on the hanger was tightened. The liquid penetrant indications were removed by hand grinding and blending the surface smooth. All indications were corrected and the areas reexamined with he same method that located them.

The feedwater nozzles were reexamined as an augmented examination. Radiographic examination was conducted as an aid in evaluation of the ultrasonic examination. The radio-graphs revealed the same indications as seen on the construc-tion radiograph, i.e., porosity and slag. Magnetic particle examinations revealed I linear and 1 rounded indication on feedwater loop B. The indications were removed by hand grinding and blending the surface smooth. The depth of the linear indication was approximately 3/32" and did not exceed the minimum design thickness of the joint. The rounded indication was also removed by hand grinding and did not exceed 1/64" in depth. All indications were corrected and I the areas reexamined with the same method that located them.

3.0 EXAMINATION OF REACTOR PRESSILE VESSEL r Westinghouse was contracted to perform the inservice volumetric I- examinations of the reactor pressure vessel. Examinations were performed using tF .ir remotely operated ultrasonic inspection device for the shell-to-flange weld (29%); ligaments (27%); one outlet nozzle's vessel-to-nozzle weld, inner radius and safe-end l weld. Manual ultrasonic and magnetic particle examinations of the reactor studs and nuts. Visual examination of the reactor studs, nuts and washers; conoseal bolting; and closure head clad-ding patches. No reportable indications were noted.

l l The examinations were performed in accordance with the require-l ments of Prairie Island's Technical Specification, Section TS 4.2 l and the requirements of the 1974 Edition of Section XI of the ASME l Code up to and including the Summer 1975 Addenda.

In addition, Westinghouse's ultrasonic inspection device was used for investigating for the presence of underclad cracking in the reactor vessel nozzle bore region. The reactor coolant outlet nozzles, Loop A and B, were examined for evidence of underclad cracking f rom the vessel nozzle protusion to the nozzle-to-safe-end weld. No reportable indications sere detected.

Copies of the procedures, inspection programs, inspection data and evaluation, and personnel certifications are contained under

, a separate report prepared by Westinghouse. This report is main-tained on file by Northern States Power Company.

. 4.0 EXAMINAIION OF SEISMIC BOLTING A program for detensioning the steam generator helicoil bolts was scheduled for implementation during this outage. The program consisted of detensioning each of the 48 bolts, retightening them to a prescribed valve, and then performing an inplace ultrasonic examination. The helicoil bolts (so named because of the heli-coil insert utilized in the base metal) form the connection be-tween the steam generator channel head casting and the support structure it rests on.

During the initial detensioning, two of the bolt heads sheared off.

An ultrasonic examination was then conducted, prior to detensioning, on the remaining bolts to determine the extent of the situation.

The ultrasonic examination revealed 26 of the remaining bolts that did not exhibit a back reflection; indicating that a similar pro-blem existed in them. Of these 26 bolts, all but one sheared off in the removal process; this one was sent to Westinghouse for analysis. All the bolts were removed and ~ replaced with a new design bolt.

Based on this information, Unit II was shut down cnd the helicoil bolts on both steam generators were ultrasonically examined. The examinations revealed 1 bolt without a back reflection, that sub-sequently sheared off during detensioning, and 2 bolts with a reduced back reflection. These two bolts were removed and a fluorescent magnetic particle examination was performed on them revealing a linear indication in the thread relief area adjacent to the bolt head. The remaining 45 bolts in Unit II were deten-sioned and reexamined ultrasonically with no further indications.

The scheduled inservice inspection program for Unit I was increased l to include 100% of the seismic bolting program. This program in-cluded all bolting, anchors, nuts and pins on the steam generators, reactor coolant pumps, accumulators, and pressurizer support struc-ture?. These were all examined visually, and where feasible, by ultrasonic examinations. The results from these examinations revealed loose nuts on both steam generator upper ring connecting bolts; a loose nut on reactor coolant pump No.11 tie back bolt; and, a loose nut and missing washer on reactor coolant pump No. 11 through anchor bolt.

All indications disclosed during the examinations were resolved prior to plant heat up. The loose nuts were tightened, the missing washer was rcplaced, and all of the helicoil bolts were l replaced with bolts of a new design. All indications were cor-rected and the areas were re-examined with the same method that located them.

This was the last inservice inspection in the Second Period of the First Ten Year Inte rval . The examinations completed the required pressure retaining components and their supports of the reactor coolant and associated auxillary systems classi-fied as ASME Class 1 and Class 2, and 100% of the Seismic Bolting program. In addition,100% of the required volumetric, surface, and visual examinations of the reactor vessel welds, studs and cladding was performed.

11.) Abstract of Conditions Noted.

The results of these examinations indicated that the integrity of these systems was maintained. The only anomalies detected were: loose bolts and nuts on Resi-dual Heat Removal Suction Hanger No. 9-RHR-l%, upper ring connecting bolts on Steam Generator No. 11 and 12, tie back and thru anchor bolts on Reactor Coolant Pump No. 11 and a missing washer on Reactor Coolant Pump No. 11 thru anchor bolts; 1 linear and 1 rounded MT indications on feedwater Loop B, weld FW-216; 9 linear and 6 rounded PT indications; 6 linear and 1 rounded,1 linear and I ro6nded, 3 rounded and 2 linear and 1 rounded on Supports A, B, C and A on Reactor Coolant Pump No. 11 and 12, respectively, an are strike on Spray to Pressurizer Hanger No.

RCRH-3/E;and cracked support heli-coil bolts on Steam Generators No. 11 and 12.

12.) Abstract of Corrective Measures Recommended and taken.

All the above indications were corrected. The loose nuts and bolts were tightened and the missing washer was replaced. The linear and rounded MT and PT indications were removed by light hand grinding and blending the surface smooth. All the support heli-coil bolts on Steam Generator No. 11 and 12 were replaced with a new designed bolt. All indications were corrected and reexamined with the same method that located them, and they will be reexamined during the next scheduled refueling outage.
