IDENTIFICATION OF OCCURRENCE: No. 3 Emergency Diesel Generator DC Control Circuit Failure 4
CONDITIONS PRIOR TO OCCURRENCE:
Normal steady power level, gross electrical output at 185 MWe DESCRIPTION OF OCCURRENCE At approximately 1650 on April 13, 1975, the auxiliary operator observed that the No. 3 diesel generator DC control circuit pilot light located outside the diesel cubicle was out.
In attempting to check the bulb he found it fused to its socket.
He then notified the control room operator of the incident, who in turn noted' that the pilot light located on the Control Room Diesel Panel was also out. An attempt was then made to start the diesel.
It failed to start.
This event is a violation of the Technical Specifications Section E.4.d,
".... Failure of one or more components of an engineered safety feature or plant protection system that causes or threatens to cause the feature or system to be incapable of performing its intended function...."
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE The appare-t cause of occurrence was a short circuit within the pilot light resultinc n a blown fuse in the DC control circuit.

ANALYSIS JF OCCURRENCE Immediately after the failure of the No. 3 diesel generator to start, the redundant diesel generators, No.1 and 2, were both started and run for 5 minutes thus verifying their operability.' This was performed to satisfy a commitment to the USAEC detailed in a letter to DOL on 2/13/73.
This verification insured that a minimum of 2 out of 3 diesel generator trains were available for operation as described in FHSR Section 411, Loss of l
Coolant Accident.
CORRECTIVE ACTION' The defective pilot light and holder were replaced and new fuses installed in the DC control circuit.
At 2039 the same day, the No. ' 3 diesel generator was test run and declared fully operational.
