Dear Mr. Madsen:

This letter is.in response to the Notice of Violation received as a re-sult of inspections conducted at this facility during the period June 1, 1980, to June 30, 1980.

t The steps taken to correct those items not in compliance, numbered in the same sequence as used in your Notice of Violation, are as follows:

1(a)

The three unsealed valves were checked to be in the open position and seals were installed.

To prevent recurrence, the valves are now secured in the required position using padlocks and chains.

1(b)

A PTR was written to have the defective pen repaired.

All

. personnel involved in chart maintenance were required to re-view-Sections 3.1 and 3.2 of ADM 28, and Operations Order No.

80-28, and notified that failure to comply could result-in disciplinary action.

This should prevent recurrence 2.

A status card, indicating that the instrument had been re-moved from service, was placed at 5:15 pm, bringing us into compliance. -All personnel involved-in this type of activity have been advised that they are not to rely on makeshift notes, but are to use the proper procedures to indicate equipment status.

Data recorded for the shifts preceeding and following those where the information was not recorded indicates that the missing data was within allowable limits since any defect causing the readings to be outside limits would not be sporadic.

To better enable the Reactor Operator to system-atically record the required data, and to act as a reminder to do so, the use of a log sheet for this purpose was imple-mented. This should prevent recurrence.

We are now in full compliance.
