- 2) AECM-80/172, 7/30/80 On July 2, 19SO, Mississippi Peuer & Light Ccepany notified Mr. F. Cantrell of your office of a Pctentially Reportable Defi-ciency (PP3) at the Grand Gulf Nuclear Station (GGNS) construction site.
The deficiency concerns contact and indication malfunctions cf Electro Switches, Seg es 2,0, Type FR-20 and GE Switch Nu=ber 53A.
Our investigation into this matter is still in progress.
Details of this investigation are discussed in the attached status report.
k'e expect to conclude our investigctica and to determine reportability under 10CFR50.55(e) and 10CFr.21 by July 17, 1981.

Description

guiB a.
Electro Switches, Series 20, Type PR-20, supplied by General Electric, showed contact and indication malfunctions during system testing.
The malfunction occurs when the switch ic turned to the STOP pcsition and released quickly.
The internal con-tact arrangement is such that the AUTO from START position contacts close rather than the AUTO from STOP position con-t a c t.e.
A similar malfunction occurs when the switch is quickly released from the START position.
Three (3) switches (No. S3A, S33, S3C) which experienced this malfunction are used in the Residual Heat Recoval (RER) System.
b.
Electro Switch, GE Switch Number S3A, contains an additional problem with the internal cen:ac: arrcngement of the switch.
The contacts are arranged such :ht: turning the switch to the STOP position would start the RER pu=p and vice versa.
II.
Pronosed Resolution Mississippi Power & Light Company is werking with the constructor to resolve this deficiency.
The fellcwing steps are being followed in this effort.
a.
Determine the extent of the daficiency; b.
Perfor= an analysis for affects en safety of operations over the expected lifetime of the plan: assuming that the defi-ciency remains uncorrected; c.
Determine the cause of the deficiency; d.
Determine and perform necessary corrective actions including actions to preclude recurrence; and e.
Determine reportability of the deficiency under 10CFK21.
III.
Status of Preposed Resolution Our NSSS supplier is performing the following actions:
a.
Coordinate with the manufacturer, Electro Switch, to determine corrective action for the conditien described in I(a), above; b.
Taken actions to replace switch S3A noted in 1(b), above; and c.
Evaluate the nonconforming cendition with Electro Switch to determine affect on safety of plant operations.
Attachmint A to AECM-81/69 Page 2 of 2 In addition, cur constructor has determined that these switches.
are not used in GGNS safety related equipment outside the Power Generation Control Co= plex (PGCC).
IV.
Reason for Delav in Final Report Our NSSS supplier has not. completed his evaluation and proposed corrective actions.
Our final report requires this input for completion.
V.
Final Report Date We expect to file our final report en this =atter by July 17, 1981.
