Dear Mr. Bauman:
Babcock & Wilcox and Bailey Controls Company have made an evaluation of the wiring errors discovered in the ECCAS for Midland.
The errors came about as a result of wiring changes made during system checkout prior to shipment.
During system construction, it is Bailey's practice to verify wiring by continuity check.
Thus, by th e time a 1
system passes checkout, both the wiring of the system and a-the performance of the system are verified. When, however, it is required to modify wiring during system checkout, Bailey procedures are unclear with respect to the need for
" ringing out" the wiring revisions before functional test i
of the change.
Several steps are being taken to correct the existing situation.
First, Bailey will initiate field changes and/
or drawing revisions as necessary to bring the Midland ECCAS drawings and hardware into agreement.
- Second, Bailey will verify the point-to-point wiring of all changes made during checkout.
Bailey has assured B&W that a proper pre-checkout wiring verification has been made.

Bailey will take steps to assure that proper verification is nade of in-checkout wiring-changes on future safety systems.
The schedule for correcting the ECCAS hardware in accordance with proper drawings is September 30, 1981.
An interim report, of the progress will be submitted to CPCO by June 15, 1981.
