References 3, 4 and 5 reported that further evaluations and corrective actions were required to completely address 79-02.

Reference 5 provided status on five remaining specific action areas.

W R Bird in a telephone communication with Mr Frank Hawkins of your office on January 30, 1981 committed to provide an update on these actions by February 28, 1981. The following paragraphs provide this status:

1.

Anchor Bolt Use Prohibition Meetings have been conducted and correspondence interchanged between Bechtel Project Engineering and the Pipe Support Design Agency in an

a. tempt to resolve the specifics of the problem.

The following specific items have occurred:

A.

The Pipe Support Design Agency has submitted two 3DDRs requesting a 3

relaxation of the pronibition on the use af expansion anchor bolts upstream and downstream of pu=ps and as currently required by the Project Engineering specification.

and the existing criteria in the specification will be retained as far as the prohibition of expansion bolts in certain cyclic lines.

B.

Essential agreement between Project Engineering and the Pipe Support Design Agency has been reached on the method of base plate analysis which considers the effects of the use of grouted anchor bolts. Final details of this analytical method remain to be accomplished.

C.

After'the resolution of the above two items, the pipe support' design changes will be passed on to the field forces who will revise and reissue revised hanger sketches with subsequent repair and rework as required.

2.

Determination of Proper Imbedment Depth The following actions are in progress:

A.

Engineering has not been able to justify the sample selection.

B.

Additional testing of imbedment depth will be performed using controlled samples.

3.

Demonstration of Achievement of Required Factor of Safety A.

A series.of tests are planned to be initiated in March 1981 at a test facility in which the relationship between bolt torque and tension and the ultimate load carrying capacity of the different size bolts will be established under laboratory conditions.

This test will include samples of concrete as used at the Midland job site.

B.

A second series of tests at the same laboratory will develop the amount of relaxation that occurs in an expansion anchor bolt over a i

period of 90 days. This series of tests will also use Midland repaired concrete blocks.

C.

Both of the above series of tests should be completed, according to current planning, by the end of July 1981.

4.

MCAR Status Bechtel's Management Corrective Action Reports numbers MCAR-31 and MCAR-34 will remain open until all required actions for them are complete and verified.

A.

A specification concerning inspection for imbedment depth of anchor bolts in other than pipe supports will be developed and inspections performed to resolve MCAR-31.

B.

The 100% reinspection of the Bechtel installed " drop in" anchors has been completed. Engineering evaluation and disposition of the inspection results is in process.

This problem identified in MCAR-34 was reported under 50.55e.

5.

Reportability Review The results of the remaining inspections and tests will be reviewed for reportability upon completion. This includes the further tests provided by Item 6 below.

6.

Additional Expansion Anchor Inspection A new specification covering the inspection of Bulletin 79-02 parameters is being prepared. This specification will cause stud-type anchors to be reinspected on a sampling basis. The reinspection will be done on anchor bolts used for pipe support base plates.

Either a final response or a status report will be provided by December 15, 1981.
