Dear Mr. MacDonald:
Certificate of Compliance No. 5939 for the G.E. Model 1500 shipping container expires on April 30, 1981. As you are aware, General Electric is preparing new evaluations for all of its shielded shipping containers.
Mc.zever, to permit continued shipment of the current restrictive allowable centents G.E.
requests that the present Certificate of Compliance be extended to April 30, 1982.
G.E. intends to complete its submittals for recertification of the i
former contents prior to the requested new expiration date.
3 A check for $150.00 for the extension fee is enclosed.
