Primary Makeup Pump removed from service due to pipe weld leak in the suction relief line.

Redundant pump (P36B) placed in service; however, pump discharge pressure degraded, due to impeller failure, after approximately 1h hours of service.

Pump P36C was returned to E.S. Standby until the following day, then was removed from ser-vice for leak repair, leaving one operable Makeup pump.

Corrective Action:

Primary Makeup Pump "C" suction relief line pipe weld leak repaired.

Pump returned to service, giving the two required pumps operable status.

Reactor Building Emergency Cooling Fan inoperable where the two remaining cooling fans were not powered from operable independent emergency buses.

Cooling fan motor failed due to lack of bearing lubrication.

Corrective Action:

Unit was brought to cold shutdown conditions as required by the conditions of the Technical Specifications Limiting Condition for Operations.

All cooling fans were inspected and repaired, as necessary.

A lubrication requirement was added to the fan pre-ventive maintenance program.

Sodium Hydroxide Tank discharge valve, CV1617, declared inoperable due to a control power fuse failure caused by a control wire being pinched and grounded to the valve operator cover.

Valve failed in the ES position (open).

Corrective Action:

Valve operator was repaired and maintenance personnel were cautioned.
