Light Company's interim response as requested by Mr. Robert Tedesco's letter dated December 12, 1980, concerning IE Bulletin 79-27, IE Bulletin 80-06 and potential design deficiencies in bypass, override and reset circuits of engineered safety features.

Our current schedule for submit ting a response to the NRC addressing the actions contained in IE Bulletin 79-27 is June 1,

1981, with any design changes or procedure modifications being fully implemented in accordance with a schedule to be submitted with our response.

The balance of this letter, Attachments 1 thru 5, is devoted to a partial response to IE Bulletin 80-06 and potential design deficiencies in bypass, override and reset circuits of engineered safety features with our complete response to these items to follow within sixty days of the date of this letter.

ATTACHMENT 1 Response to actions contained in NRC Bulletir. 80-06 entiled " Engineered Safety Features Reset Control" ITEM 1 Review the drawings for all systems serving safety-related functions at the sch'ematic level to determine whether or not upon the reset of an ESF actuation signal, all associated safety-related equipment remains in its emergency mode.

RESPONSE; Schematic diagrams for all systems identified in Attachment 2 serving safety-related functions were reviewed to determine whether or not, upon the reset of an ESF actuation signal, all associated safety-related equipment remains in its emergency mode. documents the results of the review for each 1

system. Additional information for each component will be provided in our final response to be submitted within sixty days including equipment description, equipment number, automatic signal from the engineered safety features (ESF),

actuated function / position, function / position after ESF reset, Bechtel schematic numb er and remarks. For components not in compliance with the acceptance criteria, proposed modifications or justif; cation for use as-is are documented in the form of GGNS Field Reports which are summarized in our response to Item 3.

ITEM 2:

Verify the actual installed instrumentation and controls at the facility are consistent with the schematics reviewed in Item 1 above by conducting a test to demonstrate that all equipment remains in its emergency mode upon removal of the actuating signal and/or manual resetting of the various isolating or actuation signals. Provide a schedule for the performance of the testing in your response to this Bulletin.

RESPONSE: Test procedures are being developed at this time to test each safety-related system identified in Item 1 above to demonstrate that all equipment remains in its emergency mode upon removal of the actuating signal and/or manual resetting of the various isolating or actuation signals. All testing related to this Bulletin will be completed as part of the pre-operational test program prior to Unit 1 fuel loading.

ITEM 3:

If any safety-related equipment does not remain in its emergency mode upon reset of an ESF signal at your facility, describe proposed system modification, design change, or other corrective action planned to resolve the problem.

RESPONSE: The safety-related equipment that does not remain in its emergency mode upon reset of an ESF signal is identified in the GGNS Field Reports referenced in Attachment 3.

The proposed system modification, design change or other corrective action planned to resolve the problem is summarized below:

ATTACHMENT 1 PROBLEM #1 (Field Report IB21-0013)

B21 Nuclear Boiler System-Nuclear Steam Supply Shutof f System EXISTING COVFIGURATION: Bechtel schematic E-1160-52, Rev. 2 contains logic that indicates valves E12-F060A, B; E12-F075A, B; B33-F019 nd B33-F020 will reopen upon resetting from an isoi tion signal if they were open originally.

PROPOSED MODIFICATION: General Electric is to correct this problem via a generic engineering change authorization. New switches and relays will be added via this mechanism so that af ter an isolation, positive operator action will be required to reopen these valves.

PROBLEM #2 (Field Report IE51-0003)

E51 Reactor Core Isolation Cooling System EXISTING COVFIGURATION: RCIC/RHR steam supply valves E51-F063 and F064 may not fully meet the NRC criteria as E51-F063 will automatically re-open when RCIC steam isolation Div. II is reset, if its control switch is left in the open position.

E51-F064 may automatically re-open depending upon its control switch's position when a Div. I isolation signal is resat.

E51-F064's hand switch is a "close", "stop" which can only have its key removed in "open."

In addition, there are no alarms that indicate whether the switch is in "open" or "stop."

If the hand switch is left in the "open" position, E51-F064 will autcaatically re-open after a Div. I isolation is reset.

PROPOSED MODIFICATION: General Electric is reviewing this problem on a generic basis for all BWR's.

Corrective action will be based on the resolution of this problem by GE.

PROBLEM #3 (Field Report N62-0001)

N62 Condenser Air Removal EXISTING COVFIGURATION:

Bechtel schematics E-1150-06, Rev. 10; E-1150-08, Rev. 9; and E-1173-21, Rev. I contain logic that indicate the mechanical vacuum pump-high steam line radiation signal will close the inlet valve resulting in a pump t: ip.

Since the mechanical vacuum pump hand switch has maintained contacts, the pump will restart when the high radiation signal clears. Thus, this equipment does not remain in its emergency mode upon removal of the actuating signal.

PROPOSED MODIFICATION: Redesign mechanical vacuum pump to have starting relay R-25 remain energized thru seal-in contacts rather than hand switch contacts.

PROBLEM #4 (Field Report P44-0005)

P44 Plant Service Water System EXISTING CONFIGURATION:

In the event of a loss of of fsite power and a LOCA occuring at the same time, valves P44-F067D, F054B, and F042B do not remain in the post-LOCA (closed) position after the LOCA signal has been cleared. Thus, this equipment does not remain in its emergency. mode upon removal of the actuating signal.

PROPOSED MODIFICATION:

Redesign control circuitry on Bechtel schematics E-1228-07 and -08 by adding an auxiliary relay to I

take the place of IP41R25 an Bechtel schematic E-1225-02 that requires operator action to reset the controls af ter a LOCA has occurred.

PROBLEM #5 (Field Report IP75-0021)

P75 Standby Diesel Generators l

EXISTING CONFIGURATION: The standby diesel generators do not remain in their emergency mode when the ESF LOCA actuation signal from the load shed and sequence (LSS) panel is reset, i

It is noted that the LSS panel cannot be reset unless the ESF LOCA signal from the LPCS sytem, Division 1 or RHR system.

Division 2 to the LSS panel is first reset. An example of a sequence of events that could take place would be:

1.

The LPCS (or RHR) ESF LOCA iignal has been reset and neither high drywell presse,e nor low reactor water level is present.

2.

The LSS panel is reset to allow restoration of certain loads locked out during LOCA by the LSS panel.

3.

The diesel generator will come cut of the emergency mode and trips are no longer bypassed.

If a trip had been present and bypassed prior to signal reset, the diesel generator would trip as soon as the LSS panel ESF LOCA signal was reset.

PROPOSED MODIFICATION: Bechtel is reviewing this problem and will initiate appropriate rurective action.

PROBLEM #6 (Field Report P41-)

ATTACHMENT 4 Remainder of response requested by Mr. Tedesco's Letter of December 12, 1980 to NRC Staff Position itemized as follows:

ITEM #1: The overriding

  • of one type of safety actuation signal (e.g.,

particulate radiation) should not cause the blocking of any other type of safety actuation signal (e.g., iodine radiation, reactor pressure) for those valves that have no function other than containment isolation.

ITEM #2: rhysical features (e.g., key lock switches) should be provided to ensure adequate administrative controls.

ITEM #3: A system level annunciation of the overridden status should be provided for every safety system impacted when any override is l

active.

(See R.G. 1.47.)

ITEM #4: The following diverse signals should be provided to initiate isolation of the containment purge / ventilation system:

containment high radiation, safety injection actuation, and containment high pressure (where containment high pressure is not a portion of safety injection actuation).

ITEM #5: The instrumentation systems provided to initiate containment purge ventilation isolation should be designed and qualified to Class IE criteria.

ITEM #6: The overriding or resetting of the ESF actuation signal should not cause any equipment to change position.

REQUEST: Accordingly, you are requested to review your protection system design to determine its degree of conformance to these criteria. You should repo-c the results of your review to us within 60 days of receipt of this letter, describing any departures from the criteria and the corrective actions to be implemented. Design departures for which no corrective action is planned should be justified.

RESPONSE: Systems serving applicable safety functions were reviewed for conformance with the six items contained in the NRC staff position above. The results of this review are documented for each affected system in Attachment 5 including any departures i

from these items and planned corrective actions documented on the referenced Field Reports. A table will be provided showing compliance with items 1 through 3 for all safety systems within sixty days of the date of this letter. The accident parameters, automatic signals from the engineered safety features in item 6 are to be listed in the table to be provided in sixty days for the detailed response to IE Bulletin 80-06 actions referred to in Attachment 1.
