A recent inspection was conducted on October 1, 1980 by our Region III (Chicago) office at your radiography facility located at 1300 East 92nd Street, Chicago, Illinois.
The inspection identified seven apparent items of noncompliance which are described in Appendix A.
The apparent items of noncompliance set forth in Appendix A indicate that management has paid insufficient attention to its licensed activities.
We are especially concerned that adequate training has not been provided to individuals acting as radiographers, and that there have been no internal management audits of the program to determine if the activities were being conducted safely and in compliance with NRC requirements.
Inadequate training is demonstrated by the finding that two individuals who acted as radiographers had not completed any of the NRC approved training program (except for some on-the-job training) which was incorporated into the license by reference.
Consequently, we propose to impose civil penalties in the amount of Two Thousand Six Hundred Fifty Dollars for the items of noncompliance set forth in Appendix A to this letter.
You are required to respond to this letter.
In preparing your response, you should follow the instructions in Appendix A.
The items of noncompliance were discussed with you on November 6,1980, during a meeting with James G. Keppler, Director, Region III, and members of his staff.
The enforcement options available to the NRC were also discussed at that time.
