O'Reilly Gentlemen:
The following information is submitted as a supplement to our previous response to I&E Bulletin 80-24, item 3:
On December 18, 1979, a' water spill occurred in the Unit 1 drywell.
Unit 1
was in cold shutdown dus to a
generator ground.
Personnel were given a maintenance request to repair a feedwater check valve, B21-F0108.
Instead, the personnel began work on B21-F010A.
They did not physically check the isolation valves to. insure that the valves were closed.
Therefore, when the_ hinge pin was pulled on B21-F010A, a spill occurred in the drywell (approximately 13,000 gallons).
No one was injured or contaminated as a result of this event.
It should be noted that the feedwater system in the drywell is neither an open system nor a
cooling water system as specified by I&E Bulletin 80-24.
It should also be noted that the spill occurred while the unit was in cold 9hutdown for maintenance activities and was not due to a system oeficiency.
