This is to inform the Commission that a Notice of Violation and a Not A

Dollars will be issued on or about March 3,1981, to Fort Hamilton m

Hughes Memorial Hospital Center. This action is based on alleged violations relating to failure.of equipment and to the radiation protection program.

The situation involved:

(1) failure to report malfunction of the teletherapy unif, when both the beam off/ beam on indication lights were on, as required by emergency procedures; (2) an exposure to a teletherapy technician upon entering a high radiation area without personnel dosimetry resulting.in an estimated radiation dose of 232-525 millirems; and (3) failure to have the required 5-year teletherapy maintenance performed. A news release has been prepared and will be issued about the time the licensee receives the Notices.

The State of Ohio will be informed.

It should be noted that the licensee has not been specifically informed of the Enforcement action. The schedule of issuances and notification is:
