Keene Corporation Ray Proof Division Docket Number 99900727/80-02 NOTICE OF DEVIATION Based on the result of an NRC inspection conducted on August 27-28, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 states in part, " Activities affecting geslity shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
A.
% tion IX of the QAM states in part:
" Quality Assurance shall retain records of the welder and welding procedure qualifications and the weld sample for review by client auditors.
Contrary to the above, the weld samples used for qualifying the welders performance were not available for review and inspection.
B.
Section IX of the QAM, states in part:
"All welding shall be performed by qualified and certified welders in accordance with prequalified procedures per AWS D.I.1-75...."
Contrary to the above, it was determined that two welders had made pro-duction welds, without having been properly qualified by the company.
