Babcock & Wilcox Company Nuclear Power Generation Division Docket No. 99900400/80-04 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on November 3-7, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.

Criterion V of Appendix B to 10 CFR Part 50 states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accor-dance with these instructions, procedures, or drawings.

Instructions, pro-cedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satis-factorily accomplished." Deviations from these requirements are as follows:

Authorized Administration Procedures Deviation Letter dated February 12, 1980, states in part that, as a result of the recent reorganization of NPGD Engineering, "It will no longer be possible to process engineering documents in strict accordance with existing procedures, Therefore, in order to insure continued smooth flow of such documentation, the reviews and approvals described in the attachment will replace those specified in the procedures."

Contrary to the above, twenty percent of the design documents examined during the inspection did not exhibit evidence (signatures) of review and approval by the individuals designated to review and approve design documents as committed in the attachment to the deviation letter.

Section VI of procedure NPG-1702-09 (Preparation / Review / Approval of QA Manual) and section 6 (Document Control) of the QA Manual respectively states in part that the "... QA Manual shall be revised as needed

... to ensure prompt inclusion of changes to these and related docu-i ments and to ensure proper control of obsolete or superceded documents l

to prevent their inadvertent use."

Specifically, the QA Manual was not revised to reflect an l

additional organizational unit added to the Quality Assurance organi-zation, the two reorganizations of Engineering Department that occurred during the past year, realignment of NPGD organizational functions and responsibilities, the revised preparation and review and approval require-ments for design and procurament documents, the redefined design interfaces, and references to-superceded or obsoleted documents.
