y Attachment to AECM-81/28 Page 1 of 1 STATUS REPORT NO. 1 FOR PRD-80/71 I.

Description of Deficiencv An' Architect / Engineer authorized = edification to safety related trap-door type fire dampers was_ performed. However, the Archi-tect/ Engineer failed to obtain. approval frem the manufacturer prior to making the modification. The modification involved relocating hinges and latches 90 degrees frem the designed /

manufactured position. Safety implications have not yet been determined, although the Architect / Engineer has informed MPSL that the change affects the geometry of the installation and may affect the seismic qualification of the dampers.

If the dampers were to fail during a seismic event, the operation of the Control Roc = HVAC System could be affected. The NRC Region II was notified of this problem on December 16, 1980.

II.

Approach to Resolution of the Problem The Architect / Engineer will investigate the problem in concert with the vendor and determine the actions necessary to correct the problem and to preclude recurrente. Based on this deter =in-ation, any required modifications will be made to the dampers.

III.

Status of Procosed Resolution 1.

The vendor has been contacted and requested to investigate the modification for possible affects on UL classification, seismic qualification and air flow testing and to provide the Architect / Engineer with the results for their investi-gation.

l 2.

The Constructer has begun a 100% inspection of the 391 fire dampers already installed. Inspection is expected to be complete by March 1, 1981.

IV.

Reason for-Delavine Final Report The vendor investigation has not been completed.

V.

Projected Completion Date A final report is expected to be submitted by March 25, 1901.
