O,U Tube Turns Division of Cheaetron Corporation -
NOTICE OF DEVIATION Based on the results of an NRC inrps; tion cenducted on August 25-29, 1980, it appears that one of your activities was not conducted in accordance with NRC requirements.
Criterion XVI of Appendix B to 10 CFR 50 states in part:
" Measures shall be established to assure that conditions adverse to quality such as.... defective material... are promptly identified and corrected.
In the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and corrective action taken to preclude repetition. The identification of the significant condition adverse to quality, the cause of the condition and corrective action taken shall be documented and reported to appropriate levels of management."
The QA Manual procedure TT-22-237, Revision 2 "Nonconformances and Corrective Action", paragraph 7.2, with regard to corrective action, requires that for major errors, the Superintendent-Inspection shall initiate a Corrective Action Request. Paragraph 7.5 indicates that Corrective Action Requests can be used 4
to direct Purchasing to obtain information as to the cause and action that will be taken by the vendors when their performance does not meet Tube Turns require-ments.
Contrary to the above, a Corrective Action Request was not issued by Tube Turns to Armco National Supply Company to obtain cause of and actions to preclude repetition of an identified materials mixup error by the vendor, which repre-seated a significant condition adverse to quality, i.e.
Armco mixed in J-55 alloy steel in an order for WF-02 carbon steel used by Tube Turns in ASTM A.234 WPB fittings zanufacture.
