Based on the results of an NRC inspection conducted on October 7-10, 1980, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix 3 to 10 CFR 50 states:
" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type approp-iste to the circumstances and shall be accomplished in accordance with these Listructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:

Topical Report SQ-TOP-1, Revision IA, commits in Section 3 (17.1.3),
Design Control, to following the requirements of ANSI N.45.2.11 (1974).
Section 10 (Records) requires that design records shall be legible, identi-fiable and retrievable.
Contrary to the above, the microfilm copy of Calculations No. AFW-1 (HEL3A 3reak Point Selection for the Auxiliary Feedwater Discharge System, H-633 Sh 4-7, Unit 1) and AFW-3 (HELBA Break Point Selection for the Auxiliary Feedwater Discharge System, H-634 Sh 5-8) were not legible.

Topical Report BQ-TOP-1 commits in Section 3 (17.1.3), Design Control to following the requirements of ANSI N45.2.11 (1974).
Section 4.2, Design Analyses, requires that " Analyses shall be sufficiently detailed as to purpose, method, assumptions, design input, references, and units such that a person technically qualified in the subject can review and understand the analyses and verify the adequacy of the results without recourse to the originator...
Procedures shall include requirements for:
" Identification of computer calculations, including computer type, code or programming, inputs and outputs."
Contrary to this commitment, computer calculation procedure EDP 4.37, calculations, does not contain requirements for the identification of computer code number, name, or version used in design analyses.
Siace corrective actions and preventive measures are currently in progress no written response to this item is necessary.

Paragraph 6.1 of EDPI 4.25.1, Design Interface Controls, states in part, "Each originating design group shall maintain a log of all documents which are routed to personnel external to the design group. These logs shall be retained as quality assurance records; thereby providing visibility of the projects' design interface control...."
Contrary to the above, the Design Interface Control Log is not being maintained in the Control Systems design groups.
It appears that a page is missing from this group's log for control of specifications (Volume 3), and other volumes had pages unnumbered or numbered incorrectly.
Further, the missing page from log of specifications could not be retrieved from the QA records system.
