Dear Mr. Seyfrit:
In accordance with the requirements of 10CFR50.55(e) we are hereby providing two copies of the Interim Report of Significant Deficiency No. 19 " Target Rock Motor Operated Valves - Bushing Not Staked". The final report will be submitted by July 31, 1981.
During the investigation of the failure of a High Pressure Safety Injection (HPSI). header shut-off valve at another nuclear facility, the threaded bushing was found loose. This deficiency could cause degradation in operation of the safety related HPSI and/or LPSI systems.
The threaded bushings will be corrected as specified in the attached interi= report and inspected prior to pre-op testing and commercial operation.

Backcround By telephone conversation on November 12, 1980, Louisiana Power and Light notified the Nuclear Regulatory Conmission concerning a deficiency which had earlier been identified in NRC IE Circular No. 80-Oh Section IV dated March lk,1980. Although this circular stated that no written response was required from licensees, further evaluation indicated that the deficient condition vould be reportable under the terms of 10CFR50.55(e). The subject valves were purchased from the Target Rock Co.

Description of Deficiency As detailed in IE Circular 80-Oh dated March lb,1980, an inveatigation of the failure of a High Pressure Safety Injection (HPSI) header shut-off valve at another nuclear generating station revealed that the threaded bushing was loose. The bushing was not staked or otherwise secured to prevent movement. This bushing is part of the mechanism which converts the valve stem rotation to linear travel of the disk. The full onen limit switch is rear driven by the valve operator and movement of the bushin can alter the relationship between stem rotation and linear travel such that travel in the open direction nay be terminated prematurely by the limit switch.
Analysis cf Safety Inclications If one or more of the bushings in either the HPSI or LPSI shut off valves were to loosen and back out, valve stem travel could be terminated prematurely by the limit switch, and with the valve not fully open, safety injoetion flow might then be restricted to a value less than uns assur.ed in the FSAR. Thus, there vould be degradation in Operation of the safety related HPCI and/or LPSI systems.

l As detailed in II Circular 80-Ci dated March 11, 1980, an investigation of the failure of a High "ressure Safety Injection (EFSI) header shut-off valve at another nuclear zeneratin: station revealed that the threaded bushing was loose. The bushin vas not snaked or otherwise secured to prevent movement. This bushing is part of the mechanism which converts the valve stem rotation to linear travel of the disk. The full open limit switch is gear driven by the valve operator and movement of the tushine can alter the relationship between stem rotation and linear travel such that travel in the open direction nay be terminated prematurely by the limit switch.
