NOTICE OF VIOLATION Lewis-Gale Hospital, Inc.
License No. 45-09207-01 Based on the NRC inspection October 20, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
-j As required in part by 10, CFR 35.14(b)(5), any. licensee who possesses and uses by product material for Group VI procedures shall cause each source or device containing more than 100 microcuries of by product material with a half-life greater than 30 days to be tested for contamination and/or leakage 1
at intervals not to exceed six months.
Contrary to the above, the cesium-137 sources possessed for Group VI proce-dures have not been tested for leakage and/or contamination since January 1980.
This is an infraction.
