Docket No. 99900002/80-02 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on September 9-12, 1980, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
Criterion V of Appendix 3 to 10 CFR 50 states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures, or draw-ings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instruc-tions, procedures, or drawings shall include appropriate, quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
In the area of control of nonconforming items, Sectioc 10 of the Combustion Engineering Quality Assurance Manual, dated March 1,1977, states in part, "This procedure has been established to assure that special processes, including welding, are accomplished under controlled conditions using qualified personnel, procedures and equipment Contrary to the above, in the Grid Cage Assembly area the following lack of controlled conditions was observed for some processes.

The welder making the tack weld of the Lower End Fitting to Lock Disc was found to be welding at 17 volts which was not within the range specified on P-Sheet 598, Revision 3.
2.
The Qualification Data Sheet #5 associated with P-sheet 598 did not document the amperage range found on P-598.
3.
The welder making the tack weld of the Spacer to Control Rod Guide Tubes did not use a calibrated voltmeter.
4 P-Sheet 516 Revision 3, for the same weld as above, had been changed from the Qualification Data Sheets amperage and voltage ranges without requalification of the welders as required by paragraph 7.4.3 of Pro-ccdure Mfg-08-13 Revision 1.
