f'T Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-10 Docket No. 50-237 Docket No. 50-249 Based on the inspection conducted on April 27 and 28, and May 2-5, 1978, it appears that certain of your activities were in noncompli-ance with NRC requirements, as noted below. Item I is an infraction and Items 2 and 3 are deficiencies.
1.
Contrary to 10 CFR 20.201(b), adequate evaluations were not made to determine compliance with 10 CFR 20.103 for several individuals exhibiting high whole body counts during the fourth quarter of 1977 and the first quarter of 1978.
2.
Contrary to 10 CFR 71.12(b), Type B quantities of byproduct material were delivered to a carrier for transport on several occasions during 1978, although copies of all documents referred to in the certificates of compliance were not possessed nor had NF3S been notified prior to first use of one of the shipping containers.
3.
Contrary to 10 CFR 71.62(a), Type B quantities of byproduct material were delivered to a carrier for transport on seteral occasions during 1978 without maintaining the required records.
