NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-10 Docket No. 50-237 Docket No.50-24c Based on the inspection conducted on May 3-5 and 10-12, 1777, it appears that certain of your activities were in nonc"a-pliance with NRC requirements, as noted below.
Item I is an infraction, and item 2 is a deficiency.
1.
Contrary to 10 CFR 50, Appendix B, Criterion 5, and the licensee's Quality Assurance Program, a surveillance test of the Unit 3 ECC's undervoltage relays was not accomplished as identified in special operating procedure No.185, in that the shift engineer was not natified prior to beginning the test and the final results of the test were not. clearly reported to station management. Additionally, the procedure did not identify acceptance criteria.
2.
Contrary to Technical Specifications 6.2.A, (Unit 3) on two separate occasions the licensee failed to follow written and approved procedures.
In one incident, the requirement to generate a deviation report - and another incident relating to locked valves on the standby liquid con-trol system.
