The inspector discovered that attendance records to mandatory lectures for two licensed employees did not exist.

Each of these two individuals was required to attend a lecture and take the subsequent quiz.

Discussion:

The NRC approved licensed operator requalification program requires that each individual participate in an annual examination.

If an individual scores less than 80% in any exam section he must receive a lecture and a quiz on that section.

It is the established practice of the Dresden Training Department to follow the approved requalification program and provide both the lecture and the quiz.

In this particular case, the instructor neglected to document the lecture, however, the required quizzes were in each individual's file.

Corrective Action:

The instructor was questioned regarding the lectures and to attest that they were in fact presented.

Both of the licensed individuals were questioned and they also attest that they received the lectures.

In addition, the licensed individuals have signed a written statement to document this fact.

This statement will be placed in their files.

Corrective Action to Avoid Recurrence:

All current instructors have been reminded of the necessity to accurately document training.

No further corrective action is deemed necessary at this time.

Date of Full Compliance:

Full compliance has been achieved.
