On January 29, 1981, a substantial safety hazard as defined by 10CFR21 was reported to Mr. John Rausch at the NRC Region II Office. The item involved the addition of a non-safety related instrumentation loop in the Nuclear Steam Supply System protection cabinets without installing the necessary isolation i
devices for separation. Details are given on the attachment.
The item is being reported under our 10CFR50.55(e) procedure. This is con-sidered a final report on the matter.

Nature of Defect, Substantial Safety Hazard Created, and Evaluation The architect-engineer requested the manufacturer to relocate a balance of plant (BOP) instrument loop in the NSSS protection cabinets because:
of a space problem. In making this change, the isolation normally pro-vided in the protection cabinets was not added. This lack of separation could have permitted a fault in a non-safety related control signal cir-cuit to propagate to the protection circuits.
