Dear Mr. Seyfrit:

j In response to the Notice of Violation centained in the subject report ve note once again that matters which were discussed with your inspec-tors on site are not represented in the report. We spent considerable time with your inspectors discussing our position on equipment control procedures and I personally discussed the matter with your inspectors.

Vith referene te the Notice of Violation we do not believe we are in violation of the guidance nrovided by ANSI 18.7-1972 concerning inde-pendent verification.

The ANSI Standard requires independent verifi-cation to ensure that necessary measures, such as tagging equipment,

have been implemented correctly.

We do not interpret _this to =ean that each and every tag must be independently verified, but rather that the implementation of the tagging sys tem and the procedures control-ling the tagging system are properly implemented and verified inde-pendently.

The total quantity of Auxiliary Clearance Carde issued with the clearance is recorded.

Upon return of the system to service all auxiliary tags are accounted for by the shift supervisor or the designated reactor operator independent of the operator returning the system to service.

3 Systems that are cleared for maintenance are surrendered for test, if appropriate, prior to returning the system to service.

Applicable cortions of the Technical Specifica-tion surveillance tests and/or appropriate functional tests are conducted to again independently verify that the system and/or equipment is ocerable under the provisions of the Technical Specifications or the appropriate operating requirements.

The Quality Assurance Department performs periodic surveil-lances and audits to provide independent verification that the tagging system is properly implemented.

These surveil-lances and audits are done frequently using checklists designed to verify valve lineups, control of the tagging l

process, as well as other operational activities.

In addition to the above, when the unit is returned to service after an outage the return to service is accomplished by the use of Overall Plant Operating Procedures (OPOP) which contain detailed valve and equipment lineup checklists as well as other operating prerequisites. The Overall Plant Operating Procedurec are utilised to ensure the plant is returned to service properly and safely.

Another situation peculiar to Fort St. Vrain is that our emergency core cooling systems are in normal routine operation when the plant is running.

It a system should be returned to service improperly we hate immediate indication of the situation.

tTe believe that the conbination of all of the above items provides the indecendent verification of equipment control, and that the combination of these activities meet the intent of ANSI 18.7.

Your report also indicates that Region IV feels that all repositioned valves or circuit. breakers should be controlled under the equipment control program.

As we attempted to point out to your inspectors such a centrol system would be impractical during ca.jor system outages.

Our system provides for tagging out system boundaries for personnel l

safety, but does not provide for tagging each and every valve or piece l

of equipment internal to the tagged out system boundary.

Once the system is out of service valves or equipment internal to the tagged out bounda cy are subject to repair, removal, or modification, and upon comple ion of the work, in-service leak tests, hydro tests, cold check-cut tesca, and/or functional tests must be performed.

Not only would

Of what value would it be to tag a valve ud then remove tha valve for repair.

With the number of valves, instruments, and equgment that m.y be within a tagged boundary the only practical means of independent verification vould be by functional test prior to retu'ning the systen to service.

I do not believe that Region IV gave this ratter the necessary thought it deserves prior to taking the position outlined in the subject report.

Contrary to the :Totice of Violation we do not believe we are in viola-tion of Section 5.1. 5 of A'iSI.'Il3.7-1972, and it is requested that you reconsider the :Totice of Violation.
