nortce or oevt^Tto" Based on 'the results of an NRC inspection conducted on June 25-27, 1980, it appears that certain of your activities were not conducted.in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR 50 and paragraph NCA-4134.5 of Section III to the ASME Code states:
" Activities affecting quality shall be pre-scribed by documented. instructions, procedures, or. drawings, of a type appropriate-to the circumstances and shall be accomplished in accordance 3
with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shal.1 include appropriate quantitative or qualitative acceptance criteria for detennining that important activities have been satisfactorily accomplished." Deviations from these requirements are as follows:
A.
Paragraph 2'.2.5 of RI-FCD Procedure No-36-40-27-05 states in part, " Audit the return of the Document Receipt Form such that the number of working days between issue and audit does not exceed ten (10) days for the docu-ments covered by this procedure...."
k Contrary to these requirements the required audits were not perfonned as evident by the Document Receipt Forms issued to the Pre-Assembly Inspec-tion on June 4,1980 (QAP Number 79-90301-V, Revision 5) and June 6, 1980 (QAP Number 80-90360-V, Revision 1) not being returned by the specified dates of June 18, 1980 and June 20, 1980 respectively.
Prior to the termination of the inspection the two Document Receipt Forms were signed by the recipients and returned to the quality assurance and a training session was held for the auditor and pre-assembly inspectors con-cerning the requirements of the applicable procedure.
No further response to this deviation is required.
B.
The recalibration sticker on the Krautkramer-Bransen, Inc. Model USIP-ll (Rockwell Tag No.36-757, Serial Number 2197-3459) ultrasonic test equip-ment had a recalibration date of February 1,1980.
Contrary to these requirements the ultrasonic test equipment was not re-calibrated and the nondestructive testing personnel continued to use the equipment beyond the date for recalibration.

The obsolete calibration sticker was replaced and a training session for the' nondestructive testing personnel was held and documented on June 26, 1980, in which they were instructed that ultra-sonic testing equipment should be checked for calibration prior to use, 4
